Recreational Scholarships- Scholarships for this division are handled in the office and proof of free/reduced lunch is required at time of enrollment plus payment for player. .

Competitive Forms

Drop form for NYSA competitive player (click here) This form is turned into the NYSA office by parent of player before any transfers, drops, or stopping of fees will occur.

 

Transfers within NYSA require a transfer form during the allowed periods. Please check these date on their website. Check the Frontier Country website/forms/transfer form

Transfers of competitive players between clubs-allowed only Dec 1-15 with the form also on Frontier Country website signed by coaches/parents and turned in with fee to Frontier Country.

All changes must be 72 hours before the event the player wishes to play in.
